Position: Reentry Case Manager/Housing Navigator Program Coordinator

Division/Unit: Reentry & Neighborhood Safety Programs

Salary:  $45K – $50K, Full-Time, 40 hours/week

Non-Exempt, Eligible representation by the California Professional Employees, #2345

Overview

The Case Manager will need to have extensive experiences working with individuals that are homeless and individuals returning from incarceration. The Case Manager under the direction of the Program Manager provides client advocacy, case management, benefit establishment, linkage to Mental Health/Substance Abuse services, linkage to stable housing and all other supportive services as needed. Case Managers/Housing Navigators will provide individualized client support by helping each client develop a plan to address their barriers, increase their income, and maintain and sustain permanent housing. As part of the plan, the Case Manager/Housing Navigator will identify each area in which clients will need assistance to accomplish the outlined goals and objectives (i.e. scheduling appointments, applying for public benefits, identifying subsidized housing, etc.) and the Case Manager/Housing Navigator will take full responsibility for their clients’ success.

Duties & Responsibilities

  • Service a caseload of up to 25 active participants, all of which will be adults returning from incarceration
  • Provide individual case management sessions with all clients at regularly scheduled intervals
  • Develop individual service plans for all clients that identify barriers to successful reentry
  • Document all client contact and progress, including time and date, type of contact, outcome and
    plan of action
  • Provide or broker services to holistically address clients’ needs
  • Oversee sessions with volunteer mentors, including tracking contact times, facilitating activities
    and resolving issues
  • May also perform the duties of employment training specialist and/or job developer
  • Provide regular updates to the Program Coordinator.

Minimum Requirements

  • Bachelor’s Degree in Social Work (BSW), Psychology, Counseling, Criminology or a related social/behavioral science field
  • Master’s Degree in Social Work (MSW) provides distinct advantage
  • 2-3 years experience in delivering case management services
  • Excellent verbal, written and interpersonal communication skills
  • Ability to work effectively with people of diverse educational and cultural backgrounds, from ex-
    prisoners to public officials
  • Computer literacy, including knowledge of basic software applications and familiarity with the
    internet and email communications
  • Knowledge of criminal justice system and/or experience with ex-prisoners is preferred
  • Ability to travel within city and surrounding communities

  • While performing the duties of the job, the employee is regularly required to communicate in person or by telephone. The employee must be able to travel to client locations. The employee is frequently required to stand, walk, reach, bend; use hands to finger, handle or feel objects, tools or controls; and reach with hands and arms. The employee occasionally must sit. The employee must occasionally lift and/or move up to 40 pounds.

  • While performing the duties of this job, the employee is occasionally exposed to Shelter like settings and vehicles. The noise level in the work environment is usually quiet to moderate. Employee must be willing to accept inherent risks presented by working in a shelter for the homeless setting. These characteristics are representative of those an employee may encounter performing the essential functions of the job. The employee works with seriously mentally ill individuals where a high degree of stress is possible. The position does not normally involve exposure to blood, body fluids or tissues; but it may require performing unplanned tasks where exposure may occur. The position requires the use of standard precautions.

  • Ability to solve practical problems and deal with a variety of concrete variables in situations where standardization may or may not exist. Ability to reason effectively and interpret a variety of instructions furnished in written, oral or diagram form. This position requires the individual to work with minimal supervision. Guidance is available as necessary, however, the individual is expected to be able to function autonomously and make individual decisions when appropriate. Position does require ability to interact with a variety of individuals and the ability to meet deadlines and time pressure based on the volume of work.
