Our Organization 

The mission of BOSS is to help homeless, poor, and disabled people achieve health and self-sufficiency, and to fight against the root causes of poverty and homelessness. The organization was founded in 1971 by a group of volunteers from the Hillel Streetwork Project in Berkeley who responded to the needs of mentally ill individuals being released to the streets by state hospital closures. Today BOSS is an award-winning organization that helps people who are facing deep poverty and multiple special needs. BOSS works one-on-one with each family and each individual to help them achieve stable income, permanent affordable housing, and lasting wellness.

 

Case Manager RESPONSIBILITIES/STATEMENT OF DUTIES

  • Clinically screen referrals as well as ensuring the referrals meet contractual obligations and expectations.
  • Model early engagement of all new residents to the program.
  • Engage to residents and complete mental health medical necessity based assessment and resident driven service plan within 30 days of program entry.
  • Meet at least weekly with residents to review progress on resident goals and resident community integration challenges.
  • Use written log communication as well as verbal communication to ensure that services and support needs are coordinated with other program staff.
  • In partnership with the resident, (and outside providers) develop and coordinate a housing exit plan for each resident within 60 days of program entry.
  • Help ensure integration of clinical concerns and the Wellness & Empowerment focus in program service activities and interactions is maintained.
  • Liaison with community providers regarding symptoms and or behavioral concerns.
  • Conduct or coordinate individual and group psycho-education, i.e. symptom management and medication, AOD recovery, health care issues.
  • Work collaboratively with and assure coordination among all providers (Includes benefits advocacy, income development, and money management, housing options, health and dental care, AOD treatment, social and family relationships.)
  • Ensure that Wellness & Empowerment principles are integrated into interdependent living skill training and planning groups; e.g. shared house responsibilities and accountability, personal and area maintenance, conflict prevention and negotiation.
  • Monitor and oversee budgeting and money management, and savings plan.
  • Maintain resident chart documentation as consistent with HMIS and PATH funding requirements and audit standards.
  • Support house structure and activity as needed regarding oversight of daily maintenance and inventory procedures
  • Escort residents to outside meetings and appointments as needed
  • Ensure resident compliance with lease standards and document all incidents relating to Lease, Standards and or Program structure deviations.
  •  Other duties as assigned by the Program Manager.

Case Manager MINIMUM REQUIREMENTS

Education and Experience:

Preferred:

  • Minimum BA/BS Degree with focus on social services.
  • Minimum of 3 years experience working homeless populations
  • Good understanding of diversity issues and ability to work cooperatively and respectfully with a diverse team of co-workers and a diverse client population.
  • Good oral and legible written communication skills.

Skills and Work Experience: Minimum of two years paid experience working with issues related to homelessness.

  • Knowledge of motivational strategies and Wellness and Recovery Model.
  • Ability to teach and model norms for functional community and communication behaviors.
  • History of good negotiation, conflict prevention and intervention skills.
  • Verifiable history that shows good personal boundaries and ability to use authority appropriately.
  • Flexibility and openness to try different interventions.
  • Ability to hear and integrate critical feedback as well as to give critical feedback appropriately.
  • Desire and ability to work with a racially and ethnically diverse clientele that is facing multiple challenges related to disabilities and homelessness.
  • Good writing and editorial skills.  Capable of providing written reports, and maintaining required case notes and other documentation.
  • Have good keyboarding skills and computer literacy; must be able to use internet applications, web browsers, email, search engines efficiently.
  • Possess a good driving record, have valid driver’s license and have access to reliable vehicle with insurance.
  • Have a current certification in CPR/First Aid.
  • Pass a criminal background check.

  • Crisis Intervention and Counseling: Understands and is able to utilize effective crisis intervention and counseling skills, and identifies the goals of counseling and the ethical and confidentiality issues involved in domestic violence counseling.
