The mission of BOSS is to help homeless, poor, and disabled people achieve health and self-sufficiency, and to fight against the root causes of poverty and homelessness. BOSS develops solutions to mass homelessness, mass incarceration and community violence and is dedicated to the inclusion and habilitation of people marginalized by addiction, trauma, criminality, incarceration, poverty, racism, sexism, homelessness and violence. Our goal is to reduce poverty, inner-communal violence, incarceration and recidivism with tangible results that transforms the intergenerational landscape in underserved and under-resourced neighborhoods and with individuals impacted by multi-generational poverty, trauma and racism.


Job Responsibilities:

The Case Manager/Housing Navigator will need to have extensive experiences working with individuals that are homeless and individuals returning from incarceration. The Case Manager/Housing Navigator under the direction of the Program Manager provides client advocacy, case management, benefit establishment, linkage to Mental Health/Substance Abuse services, linkage to stable housing and all other supportive services as needed.

  • Service a caseload of 25-30 active participants, all of which will be adults returning from incarceration
  • Provide individual case management sessions with all clients at regularly scheduled intervals
  • Develop individual service plans for all clients that identify barriers to successful reentry
  • Document all client contact and progress, including time and date, type of contact, outcome and plan of action. 
  • Provide or broker services to holistically address clients’ needs.
  • Oversee sessions with volunteer mentors, including tracking contact times, facilitating activities and resolving issues
  • May also perform the duties of employment training specialist and/or job developer

To be considered for the Reentry Case Manager/Housing Navigator position, candidates must meet the following criteria:

  • B.A in counseling, social work or psychology. Master Degree in Social Work Preferred
  • 2-3 years’ experience in delivering case management skills. Knowledge of criminal justice system
  • Working in a community setting providing services for person reentering the community from incarceration with histories of homelessness, psychiatric disabilities, and substance abuse
