Salary Range: $70,000/Annually

Job Summary: 

The Clinical Case Manager will provide mental health and supportive case management services to participants BOSS housing residents.  Services include clinical assessments, treatment planning, individual therapy and rehabilitation sessions, collateral contacts, referrals, and case management. The Clinical Case Manager is responsible for working as part of a team and provides direct services to participants as well as collaborates with other agencies and professionals. Provide direct services, work independently and have the ability to work well with other agencies such as the courts, jail, attorneys and community partners.

Essential Duties/Responsibilities: 

  • Responsible for the assessment of Severe and Persistently Mentally Ill adults and development of an individualized, collaborative, strengths-based care plan for each assigned participant.
  • Maintain appropriate case documentation and other contract guidelines.
  • Provide direct services as outlined in the individual care plan.
  • Provide individual rehabilitation and/or therapy services in the office and community based settings.
  • Work to meet participant’s needs in the areas of: activities of daily living, household management, financial literacy, budgeting, money management, banking, safety awareness, access and use of community resources, entitlements and public assistance, access to medical, dental and wellness services, mental health medication and illness self-management, which involves developing a plan with the resident to help monitor their symptoms, track early warning signs, develop coping skills and prepare a plan to minimize and prevent relapse.
  • Provide crisis intervention which may involve Mobile Crisis Team, police or other emergency workers.
  • Assist participants with Medi-Cal, SSI/SSDI application process and MAA/TCM contacts.
  • Support participants in finding other housing, as needed or desired.
  • Attend and participate in team meetings and case consultations.
  • Attend supervision meetings.
  • Attend job related conferences and in-service training and seminars.
  • Collaborate with the referring agent for specific cases.
  • Comply with all BOSS policies and procedures.
  • Other duties as assigned

Requirements (MQs):          

  • Minimum of 2 years in behavioral health leadership position
  • Master’s degree (prefer doctorate degree) in psychology or social work
  • Licensed in California to practice as a clinical psychologist, social worker or nurse practitioner
  • Experience in a forensic discipline preferred
